Extracorporeal membrane oxygenation support in management of severe respiratory failure secondary to 2009 influenza A(H1N1) virus.

چکیده
We read with great interest the recent article by Miller and colleagues (April 2010) 1 reporting their experience with 47 patients who were admitted to the ICU with 2009 infl uenza A(H1N1) [A(H1N1)] virus infection in Utah. The authors stated that the severity of illness and mortality (27%) in their cohort who received usual care for ARDS (ie, low tidal volume, low pressure ventilation) was similar to that demonstrated in the Australia and New Zealand series using extracorporeal membrane oxygen (ECMO). 1

Extracorporeal membrane oxygenation in the context of the 2009 H1N1 influenza A pandemic.
BACKGROUND Extracorporeal membrane oxygenation (ECMO) incorporates surgical techniques as adjuncts in the management of refractory respiratory dysfunction. For many years, its primary application was for support of neonatal infants in cardiorespiratory failure. As the 2009 H1N1 influenza A pandemic developed, more reports came in of severe respiratory dysfunction and even death that seemed to b...

The most common cause of death due to the H1N1 subtype of influenza A virus (swine flu) in the 2009 to 2010 epidemic was severe acute respiratory failure that persisted despite advanced mechanical ventilation strategies. Extracorporeal membrane oxygenation (ECMO) was used as a salvage therapy for patients refractory to traditional treatment. At Legacy Emanuel Hospital, Portland, Oregon, the epi...
متن کاملVirus-associated hemophagocytic syndrome as a major contributor to death in patients with 2009 influenza A (H1N1) infection
INTRODUCTION Virus-associated hemophagocytic syndrome (VAHS) is a severe complication of various viral infections often resulting in multiorgan failure and death. The purpose of this study was to describe baseline characteristics, development of VAHS, related treatments and associated mortality rate of consecutive critically ill patients with confirmed 2009 influenza A (H1N1) infection and resp...
متن کاملNonventilatory strategies for patients with life-threatening 2009 H1N1 influenza and severe respiratory failure.
Severe respiratory failure (including acute lung injury and acute respiratory distress syndrome) caused by 2009 H1N1 influenza infection has been reported worldwide. Refractory hypoxemia is a common finding in these patients and can be challenging to manage. This review focuses on nonventilatory strategies in the advanced treatment of severe respiratory failure and refractory hypoxemia such as ...
متن کاملPostvaccination Influenza 2009 H1N1 Respiratory Failure Requiring Extracorporeal Membrane Oxygenation
The spread of pandemic Influenza A (H1N1-2009) was believed to have been attenuated by the effectiveness of worldwide vaccination initiatives. Despite the immunogenicity of a safe vaccine, we report a case of vaccine failure resulting in catastrophic influenza-associated respiratory failure.
